A delightful woman in her early 80’s who still works full time. She has a basal cell carcinoma of her nose, and her case is made more complicated by smoking 1-2 packs of cigarettes per day. Smoking can damage the blood supply of the skin and increases the risk of wound healing complications.
Her defect after Moh’s surgery is shown. This was accomplished the day before my procedure. Under local anesthesia with sedation she had a Reiger flap (sometimes called a “bishop’s mitre flap”) of muscle and skin to preserve the blood supply. This is advanced in a “v-y” fashion. It corrects the defect with skin that matches in color and texture. It also elevates the tip of the nose slightly, which in her case is beneficial and has actually improved the appearance of her nose. She is shown 5 months after surgery. Her scars will typically continue to fade and improve further over the next 6 months. A nice result in a challenging case.

A challenging case of a 17 yo student with a severely deviated nose. An “extracorporeal septoplasty” was performed. This is done in a case where the deviation is so severe that is cannot be corrected properly without completely removing the septum, rebuilding it in the operating room and replacing it in the nose. He is shown before and again, just 6 weeks after his procedure. An open rhinoplasty approach was used with a small incision placed across the columnella. He is thrilled with the natural appearance of his nose after surgery and the improvement in his breathing.

Update: Our patient is now seen 2 ½ years after his surgery. His breathing and aesthetic correction remain excellent.
