A man in his early 70s with a basal cell carcinoma of his upper lip that has started to ulcerate.  He is shown before surgery, immediately after Mohs excision by Dr. Gary Mendese, one week after peri alar bi pedicle flap repair by Dr. Jonathan Hall, and 6 weeks after the repair.

A woman in her early 40’s with drooping and stretching of her earlobes.  Her studs are now falling out, especially on her left.  After discussions, she has a nickel allergy and has been sleeping with her earrings in.  She is shown before and again 3 months after an earlobe lift combined with a split repair.  Her earlobe contour has improved, and we were able to re-pierce her ears in the office.

An 80-year-old woman with obstruction of her upper visual field from excess skin and drooping of the brows ( brow ptosis)  She is shown before and again, nearly 6 months after bilateral browpexy combined with upper eyelid blepharoplasty.  Her vision has improved and she loves her new appearance.  A browpexy anchors the cut edge of the lateral orbicularis muscle to the orbital rim periosteum and prevents the brows from dropping down.  It can be a good compromise for patients with brow ptosis who do not wish a complete brow lift procedure at the time of upper eyelid blepharoplasty.

A woman in her mid-60s with a complete split of her right ear.  She has a history of redness at the site of her earrings on both ears, suggesting a nickel allergy.

She also has a drooping appearance of the right ear.  She is shown before and again, 3 ½ months after a split earlobe repair combined with an earlobe lift.

An 80 yo woman with a basal cell carcinoma of her right ear was shown immediately after Mohs excision by Dr . Donald Grande, and then 6 weeks after a “revolving door flap” performed by Dr. Jonathan D. Hall.  The flap keeps its blood supply and is “revolved through” into the front of the ear from behind the ear, closing the donor site behind the ear and using the flap to reconstruct the defect in the conchal bowl in one stage.

A woman in her early 20s with a second split of her left ear.  She had this repaired when she was 10 at the Children’s Hospital.  It has gotten red on occasion and has split again.  We expect that his chronic inflammation is likely a sign of a nickel allergy.  She is shown before and again, 4 ½ months after repair under local anesthesia in the office.  We recommend that she avoid nickel and will re-pierce her ear with titanium earrings.

The Bilobed flap allows us to transfer skin from where we have some excess along the dorsum of the nose to the tip of the nose, using two flaps or “lobes”.  It typically provides the best color and texture match on the nasal tip and is more durable when a patient requires postoperative radiation due to the aggressive nature of the tumor (peri-neural involvement) as was the case here.  Her 4 ½ month view shows that the scars have settled nicely and she can pass the “little girl test”.

An RN  in her mid-20s with a partial split and drooping of her right earlobe, most likely from sleeping with her earrings in place.  She is shown before and again, 3 months after an earlobe lift/repair.

A woman in her mid-30s with a partial split of her left ear which she thinks happened from wearing her earrings while sleeping.  She is bothered by her earring hanging “funny” on the left.  She is shown before and again, 3 months after left split earlobe repair, and has been using topical silicone for the past 6 weeks to help with scar maturation.
