65 yo woman shown before and 4 months after short-scar facelift with micro-fat grafting to lower eyelids and naso-labial creases, bilateral lower lid blepharoplasty and chin implant.

Our patient is a nurse in her middle 60’s who has lost 60 lbs following a gastric sleeve. She has some difficulty with excess upper eyelid skin which is starting to block her vision. She has had an upper lid blepharoplasty when she was in her 30’s. She also has “festoons” of her lower eyelid skin. Her chin is slightly weak. When the chin is weak and we are considering a facelift, we will frequently add a chin implant at the same time. She is shown before and again, 5 months after an endoscopic brow lift. Upper and lower eyelid blepharoplasty, facelift and chin implant. She is thrilled with her natural appearance after surgery.

A woman in her mid to late 60’s who is bothered by an aged appearance. She is shown before and again, 5 months after an endoscopic brow lift, upper eyelid blepharoplasty, fat grafting to the cheeks and lips, Facelift with lateral Smasectomy and peri-oral dermabrasion. Her results will be limited somewhat because of decreased skin elasticity- the “fabric” that we are working with. Nonetheless, she has a dramatic and natural appearing improvement at 5 months. Skin quality is somewhat better with the fat grafting. The dermabrasion is a tried and true technique that can give improvement of the lines around the mouth but with less risk of bleaching that can be seen with laser or deep chemical peels.

A woman in her mid 60’s who is shown before and again, 6 1/2 weeks after facelift with lateral Smasectomy and perioral dermabrasion. Dermabrasion is a light sanding of the lines around the mouth which heals in 7-10 days and improves the appearance of lip lines with less chance of a permanent lightening of the skin which can be seen with laser or deeper chemical peels. She is pleased with the natural appearing improvement that is not overdone in appearance. This is an early followup.

A woman shown initially in her mid 40’s who is troubled by facial aging. She underwent a coronal (open) brow lift with bilateral upper eyelid blepharoplasty with lateral sulcoplasty(special suturing to better define the lateral eyelid crease) bilateral transconjuctival (through the inside of the lower eyelid) lower eyelid blepharoplasty with bilateral lateral canthopexy (tightening the corner of the lower eyelid), a facelift with lateral smasectomy (removing a strip of the covering over the facial muscles and tightening it with internal sutures) and micro fat grafting to the face. She is shown 3 months after surgery. Her natural appearing results give her the appearance of a younger sister, without the stigmata of surgical deformity. She did well with her plastic surgery, and returned 10 years later when she began to develop some more looseness of the neck and lower face.

A woman in her early 70’s who is shown before and again, one year after a facelift with lateral SMASectomy combined with a small chin implant and fat grafting to the mid face. As we age, it is not “gravity” as commonly thought, but a loss of volume. We are sinking in like a peach left on the counter too long. So the most natural results come from a combination of lifting and filling. If we try and correct this only by pulling, an unnatural look can result. In addition when the chin is slightly “weak” a small chin implant can improve the results. It is interesting to see that the “bags” under the eyes are improved but no surgery was performed to remove fat- only micro fat grafting to replace some of the volume lost with aging. My patients typically tell me that they want to look like a better version of themselves, not someone else. I always like to have a patient bring in a photo of when they are younger and they liked the appearance of their face as a guide.
