A 21 yo with breasts that are overly large for her frame who wears a 36 DDD bra and has difficulty exercising, back and shoulder pain, irritation of her shoulders from the bra staps and difficulty with her posture that makes it difficult to perform office work.
Her case is made more challenging by the large size of her areolas, that would fall outside of the normal pattern for breast reduction. A Short Periareolar scar inferior pedicle (SPAIR) breast reduction was performed with the removal of more than 500 gms of tissue from each breast to reduce her to a C cup. One benefit of this technique is the absence of scars along the inner breast, as demonstrated with her arms up. The lower scar has a “J” shape rather than a “T” shape. She is shown 1 year after surgery. Her scars will typically continue to fade and improve further over the next year. Her symptoms related to the large size of her breasts have all resolved.
